When most people think about plastic surgery in Miami, they imagine high-volume practices focused primarily on results. Dr. Alexander Earle of Pure Plastic Surgery understands the importance of results, but as a new docuseries highlights, his practice values safety above all.
“Safety is the foundation, and everything else is what’s built on that foundation,” Dr. Earle explains. “We are a patient-centric, safety-first practice. This guides everything we do.”
It’s easy enough for those in the plastic surgery field to say that patient safety matters, but Pure Plastic Surgery’s focus on safety is reflected in everything its professionals do.
“In practical terms, it means we are willing to do things safely, even if that means it’s less efficient or less profitable,” says Dr. Earle. “This drives careful patient selection, transparent conversations about what should or shouldn’t be done, our strict protocols, continuous staff training, rigorous facility-accreditation processes, close post-op monitoring, and the ability and willingness to say no when something is not in the best interests of our patient.”
Pure Plastic Surgery is based in Miami, which is sometimes described as the “plastic surgery capital” of the United States.
“Before Pure, there were two standards in Miami: the high-volume practices that are dubbed ‘chop shops’ and the boutique practices,” Dr. Earle says. “Women and men had two choices; that’s it. If they didn’t have a $50,000 surgery budget, their only alternative was one of these higher-volume clinics.”
Dr. Earle was a surgeon at one of those “chop shops” before starting Pure. He made the switch after seeing firsthand how many clinics refused to make patient safety a priority.
“I saw the cutting corners, the lack of transparency, and the overall ‘profit-only’ drive,” he reflects. “I always thought to myself, ‘There has to be another way.’”
Through Pure Plastic Surgery, he forged that better way himself. Teamwork, a family-driven culture, and an emphasis on patient safety undergird the way the practice operates. In building it with intention, Dr. Earle started Pure with Mikele Earle, his wife and the CEO, and has carefully selected talented individuals with similar values and priorities to join the team, including two additional surgeons, Dr. Wegerif and Dr. Vidal.
And now, a new six-part docuseries is offering prospective patients (and everyone else) a glimpse into Dr. Earle’s mission to redefine plastic surgery. The series launched on April 1, 2026, with a new episode airing each week. The second episode, “Pure BBL Ultrasound Revolution,” is of particular interest. It gives viewers a behind-the-scenes look at Dr. Earle’s approach to the Brazilian butt lift (BBL), a popular but potentially dangerous procedure.
A BBL involves taking fat from other areas of the body and transplanting it into the buttocks. It may sound benign, but if adipose tissue is injected into the gluteal muscles (as opposed to just under the skin), a patient could develop a potentially lethal fat embolism.
Dr. Earle is one of the plastic surgeons pioneering the use of ultrasound technology during BBLs. The ultrasound provides a clear picture of the area so the surgeon can make sure the fat is going exactly where it’s supposed to.
The docuseries features more than just the surgeries themselves. Viewers can witness the patient’s full journey while also coming to understand the process from a surgeon’s point of view.
Dr. Earle hopes that the docuseries will encourage anyone thinking about surgery to look closely at any practice they’re considering.
“The philosophy behind it matters. Are patients treated like individuals or transactions? Are standards actually upheld? In my experience, the best practices operate with intention,” he says.
“You’re not just selling surgery. You’re building a space where patients are heard, feel safe, and are cared for.”
